If your request is an emergency, please call us at (727) 938-3374 and press "2" for Support. If you would prefer not to submit this form electronically, please download the PDF version and fax it to us at (727) 499-7814.

Service requests are handled in the order in which they are received with priority given to EESi Maintenance Customers. Online service requests are EESi's preferred method of requesting support services. Please be aware that requests from non-maintenance customers are subject to time and materials charges at current rates.
